Service Waiver and Release
I understand that participation of this activity involves inherent risks, including but not limited to physical injury, pain and material damages. I represent and agree that to the best of my knowledge I have no impairment or ailment such that active exercise may be harmful or detrimental to my health. I also agree to disclose all health conditions that may affect or impair my ability to participate in strenuous physical activity and will seek professional medical help if advised.
I acknowledge that I have carefully read this "Service Waiver and Release" and fully understand that it is a release of liability. I expressly agree to waive, release, and discharge any and all liability and legal rights including the right to sue, Figure Fitness LLC, and its owner and trainer(s).
Cancellation Policy
I agree to the cancellation policy in which I will be charged in full for a session amount if I cancel a session within 24 hours prior to the session without intentions of rescheduling within the following 48 hours. This condition will only be subject to exception in select situations.
By reading and acknowledging this agreement, I represent that I understand the cancellation policy and the expectations that I am held to in regard to scheduling and time management.
